As an Admissions Nurse, you focus on one patient at a time , completing thorough assessments without the pressure of juggling multiple visits or ongoing caseloads.

What You'll Do:
• Complete Start of Care (SOC) admissions in patients' homes.
• Conduct detailed clinical assessments and OASIS documentation .
• Initiate care plans and ensure a smooth transition into services.
• Educate patients and families on what to expect moving forward.
• Coordinate with the care team to set patients up for success.

What You Bring:
• Active Minnesota RN license .
• Experience with home health admissions and OASIS preferred.
• Strong time management and independent work style .
• Comfortable working in a field-based, patient-centered role .
• Reliable transportation.

Additional Details:
• Mileage reimbursement provided.
• Territory-based scheduling across the Twin Cities Metro.
