Our client seeks a Nurse Practitioner to join their growing team around Elk Grove, CA. The organization partners with nephrologists throughout California. Nurse Practitioners provide hands-on case management and clinical education to prevent hospitalizations and the need for dialysis. Compensation Base pay range: $145,000/yr - $160,000/yr. Final pay is based on skills and experience. Responsibilities Travel to local nephrology clinics 3-4 days per week. Perform administrative work and provide telehealth services from home 1-2 days per week. Perform comprehensive physical evaluations. Develop individualized care plans and ensure appropriate follow up through the care continuum. Work collaboratively with an interdisciplinary team including nephrologists, RNs, dietitians, cardiologists, etc. Provide coordinated care services between the patient, family, physicians and other care team members.
