JPNDC's Family Childcare program (FCC) is a system of 115+ independent licensed early educators offering childcare in their homes to low-income children with public childcare subsidies. JPNDC supports educators in providing high-quality early education and care designed to prepare children for success in school and life. We also support families through an array of wraparound services and enrichment activities.

The Role: The Sr. Case Manager will join the Family Child Care team, reporting directly to the Associate Vice President of Community Wellness. As a dedicated and skilled professional, the Sr. Case Manager will serve as the primary liaison for our priority populations, including DCF, young parents under 24, and families experiencing homelessness. In collaboration with various agency partners, the Sr. Case Manager will support individualized family needs, facilitate tailored Family Plans, and conduct developmental evaluations. All FCC program staff members are considered mandated reporters and will be supported with the requisite training as needed. This role is ideal for an experienced social service professional who is passionate about making a positive impact in the community by supporting a staff and the families and children in the FCC program, particularly those with high needs facing challenging circumstances.

Duties:
  • Primary liaison for our priority population such as: DCF, Young Parents Under 24, and Families Experiencing Homelessness.
  • Supervise and attends regular meetings with DCF, Shelter Systems or other family service agencies.
  • Support the contractual needs of the children and families including referrals to community resources and multi-agency reporting as required.
  • Develop strategies for cross department referrals with JPNDC Economic Prosperity Services to develop Individualized Family Plans and goal setting plans tailored to each family's needs and goals.
  • Oversee staff and perform developmental evaluations such as: ASQ's/ASQ-SE's for newly enrolled and families as needed.
  • Facilitate and supports staff in parent workshops, education, self-sufficiency, and other related enrichment activities for priority population.
  • Provide families with high level and ongoing support in navigating referral processes such as: Early Intervention, Public School Systems, Public Assistance Benefits, and/or Hospitals.
  • Provide one-on-one assistance and referrals to senior staff to help identify high-needs families.
  • Conduct routine home visits to educator programs and prepare related reports.
  • Other duties as assigned by supervisor.

Qualifications:
  • Required :
    • Active LCSW or LSW license.
    • Bachelor's degree in Social Work or a related Human Services Degree.
    • Fluent in English and at least one additional language: Spanish, Haitian Creole, or Cape Verdean Creole (written and verbal proficiency required).
    • Minimum 3years direct service experience (1+yr supervisory) in one or more of the following: working with families of underprivileged socio-economic backgrounds working with multicultural populations meeting diverse family needs, stabilizing challenging circumstances, & supporting children and families who have experienced trauma.
    • Minimum 3 years of direct service experience in early education programming/administration.
    • Ability to nurture relationships across services and resources agencies.
    • Sensitivity to and experience with handling confidential information and interactions, interacting in a culturally appropriate and responsive manner with a wide range of individuals.
    • First Aid and CPR certified (certification training available).
    • Demonstrated knowledge of DEEC policies and regulations, Quality Rating Improvement System (QRIS) and early education credentialing programs.
    • Ability to work a flexible schedule with occasional evening and weekend availability
    • Valid driver's license and reliable personal vehicle required for daily work-related travel
  • Preferred:
    • Master's Degree, LCSW in mental and/or behavioral health or related certification.
    • 5+ years in Early Childhood Education, Child Development or related field.

    • Highly computer proficiency in Microsoft Office and online internet CRM and virtual meeting platforms. Must be familiar with CCFA, EEC and DCF systems and reporting requirements.

Employment Terms/Compensation : This is a full-time, salaried position with a generous benefit package. Salary range is $85,000 - $95,000.
